A cracked tooth is one of the most challenging problems in dentistry — not because cracks are rare, but because they're often invisible on x-rays and behave unpredictably. A hairline fracture can sit silently for years, then suddenly cause sharp pain when you bite down or release pressure. Left untreated, cracks tend to deepen over time, eventually reaching the pulp and causing infection or, in the worst cases, splitting the tooth beyond repair. Catching a crack early and treating it correctly is often the difference between saving the tooth and losing it.

Not all cracks are the same. Craze lines are shallow surface fractures in the enamel and usually need no treatment. A fractured cusp — where a piece of the chewing surface breaks off — can often be repaired with a crown. A true cracked tooth, where the fracture extends from the chewing surface toward the root, is where endodontic treatment becomes critical: if the crack has reached the pulp, a root canal followed by a crown can stabilize the tooth and relieve the pain. A split tooth, where the fracture has fully separated the tooth into segments, generally cannot be saved. A vertical root fracture, which starts at the root and travels upward, almost always requires extraction. Knowing which type you have is the first and most important step.

Treatment for a cracked tooth focuses on two goals: removing the source of pain and preventing the crack from progressing. After a thorough exam — often using magnification, transillumination, and sometimes a 3D cone beam scan to map the fracture — the endodontist determines whether the tooth can be saved. If it can, a root canal removes the inflamed or infected pulp, eliminates the bite-triggered pain, and seals the inside of the tooth. A crown is then placed by your general dentist to hold the tooth together and distribute biting forces away from the crack. With early diagnosis and proper restoration, many cracked teeth can function normally for years.
