An apicoectomy is a minor surgical procedure used to save a tooth when a previous root canal hasn't fully resolved an infection. The name comes from the procedure itself: "apico" refers to the apex, or tip, of the tooth's root, and "ectomy" means removal. During the procedure, the endodontist removes the very tip of the root along with any surrounding infected tissue, then seals the end of the root to prevent bacteria from re-entering. It's typically recommended when a standard root canal or retreatment isn't enough to clear persistent infection — often because of complex root anatomy, narrow or branching canals, or inflammation that has settled into the bone surrounding the root tip.
The procedure is performed under local anesthesia and is generally well tolerated. After numbing the area, the endodontist makes a small incision in the gum near the affected tooth to access the bone and root tip. The infected tissue and the root tip are carefully removed, the end of the remaining root is cleaned and sealed, and the gum is closed with a few small sutures. The entire procedure usually takes between 30 and 90 minutes, depending on the location and complexity of the tooth. Most patients return to normal activities within a day or two, with mild swelling or discomfort that's easily managed with over-the-counter pain relievers and a short course of rest.
An apicoectomy is often the last opportunity to save a tooth before extraction becomes the only remaining option. When successful — and the success rate is high in skilled hands — it eliminates the source of infection, allows the surrounding bone to heal, and preserves the natural tooth for many years to come. Saving a natural tooth is almost always preferable to replacing it: it maintains your bite, protects the surrounding bone structure, and avoids the cost and complexity of implants or bridges.
